Structured Family Caregiving (SFC)
We will help you throughout your entire process
Structured Family Caregiving (SFC) provides families with the ease that their loved ones are being cared for in a loving and safe environment. Family members can get a weekly stipend of up to $420 weekly to provide care for their family member who is eligible for the Medicaid Waiver program specifically the CCSP or SOURCE. Receive ongoing support from a health coach, health education and training, communication through online portal, and the peace mind of a Registered Nurse.
Once you are approved with SFC, you then will work with Dalia Precious Memories LLC as your family member’s provider and you as the primary caregiver for your loved one and will receive a weekly stipend that is deposited directly into your account. You will need a background check and 8 hours of yearly training that we will assist with each year.
Qualification for the Medicaid Waiver member includes:
- Being 18 years or older
- Currently have Medicaid Waiver eligibility
- They require at least 5 hours of daily personal care services (nursing home level of care)
- Must live with family member who provides the hours of care and lives in the same home/same address
Qualification of Family Caregiver:
- They are at least 18 years old
- Being related to the CCSP or SOURCE member (Medicaid Waiver)
- Not a spouse, legal guardian, or conservator of the family member for whom the care is for
- Lives in the house with the family member (same home same address)
- Pass a background check
- They cannot run a business insidethe home as well as no employment outside of the home.
